Silverdale Medica Limited is a primary health care company that operates four clinics in the Hibiscus Coast Community of Northern Auckland.
Under the community trade names of Silverdale Medical, Millwater Medical, Weiti Creek Medical and Skin Safe the company provide primary care clinical medical services.
We collect personal information from you, including information about your:
- Identify information including name, date of birth, ethnicity, immigration status.
- Personal contact information including home address, phone, email, and text.
- Employer information.
- Private insurance, billing, or purchase information.
- Personal medical history, and family medical history.
We collect your personal information:
- To provide health care to people who seek our service.
- To refer patients to services if we do not offer the service they need.
- To manage the clinical effectiveness of our service delivery over time.
- To invoice patients, insurance companies and government agencies that pay for services rendered, sometimes employers as funders where appropriate.
- To monitor and maintain health into the future.
- To recommend service to the patient that is clinically evidence based recommended interventions.
Besides our staff, we share this information with:
- Insurances companies and employers who pay some or all the cost of services rendered.
- ACC and Health New Zealand to be paid for some or all or the cost of services rendered.
- other health service providers to refer patients for health services we do not directly provide.
- auditing, and professional bodies that govern health professions to meet legal compliance or audit compliance requirements.
Providing some information is optional. If you choose not to enter identifying information, past health information, health risk information, we'll be unable to provide safe medical service..
- If you choose not to enter employer information, we will be unable to bill the employer or ACC for medical service where appropriate.
- If you choose not to enter past or family medical history, we'll be limited in the quality of medical service we can provide.
- If you choose not to enter personal contact information, we'll be unable to provide medical service.
- If you choose not to provide immigration and/or citizenship status, we will be unable to enrol you in government supported medical service.
We keep your information safe by using two factor authentication and access to only employed and contracted workers who are screened by police vetting and who have provided professional references that we verify.
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
